Key tasks: Reporting to the Program Director - Chronic Disease & Wellbeing Impact, this role is responsible for: - Providing clinical nursing care and interventions/or individual case management within scope of practice.
- Lead and support the nursing team with appropriate clinical prioritisation.
- Apply clinical knowledge and networking to connect with regional and remote areas, offering expert services in community environment.
- Provide clinical leadership to a team of nursing professionals working across Parkinson’s/Stroke and Arthritis entities. This includes the provision of clinical supervision, and mentorship, as required.
- Contribute specific expertise to monitor and evaluate research activities to improve nursing practice and service delivery.
- Lead and contribute to the development of educational resources and nursing skills to best meet the needs of consumers.
Key Stakeholders / Working Relationships - Client and Patient Services Leadership Team.
- Nurses, Occupational Therapists, Client Wellbeing Practitioners, Exercise Physiologists.
- Client Service Coordinators.
- Other employees of THRF Group.
- Other key external stakeholders.
Qualifications / Knowledge / Experience - Registered or eligible for registration as a Registered Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- At least 5 years of professional experience and competence in relevant field.
- Demonstrated experiences in clinical leadership within a healthcare setting.
- Experience in facilitating Quality Improvement Projects.
- Knowledge and understanding of public and private health systems, relevant legislation, The National Quality Health Care Standards, codes, ethics and competency standards.
- Knowledge of relevant programs such as National Disability Insurance Agency, My Aged Care, Community Home Support Program (desirable).
- Project management/experience delivering Grants (desirable).
