THRF Group Arthritis (a charity of THRF Group), are seeking an experienced Registered Nurse to join our dedicated team in the role of Nurse Specialist. This is an exciting opportunity to step into this brand-new role where you'll have the opportunity to shape and establish specialised clinical care to people living with Arthritis in the community. We’re looking for someone who has: - Current AHPRA Registered Nurse with a minimum 5 years’ post graduate experience
- Experience and/or interest in chronic disease management, rheumatology and related musculoskeletal conditions
- An understanding of primary and tertiary health care
- Ability to work effectively both independently and as a member of a multi-disciplinary team with a flexible positive attitude.
In this role you will: - Establish and deliver specialised clinical care and education for people living with Arthritis in the community, shaping the development of this new service.
- Champion self-management principles to empower clients in setting and achieving health goals
- Work collaboratively within multidisciplinary teams and leverage networking skills to liaise with external providers, ensuring seamless and integrated care pathways.
- Actively promote access and equity of services for diverse populations, including those in regional and remote areas, ensuring all services are culturally sensitive.
- Engage in continuous professional development, reflective practice, and contribute actively to quality improvement initiatives, service evaluation, and resource development.
