THRFG - Parkinson’s team provide a variety of services and programs to improve the quality of life for those living with Parkinson’s and other movement disorders.  Our Parkinson’s Nurse Specialist’s provide up to date, evidenced based information and support services to people living with Parkinson’s and other movement disorders, their carers and family within a community setting.

 

About the role

THRFG Parkinson’s are seeking an experienced Registered Nurse to join our dedicated team as a Parkinson’s Nurse Specialist, working .6FTE, providing individualised support to people living with Parkinson’s Disease in the community. Delivering specialist clinical care including case management, advocacy and provision of clinical education within their scope of practice.

 

The role delivers safe, effective, and person-centred care, underpinned by evidence-based practice and aligned with relevant accreditation standards. Working with a high level of autonomy, the role contributes to service evaluation, reporting, and continuous quality improvement to ensure excellence in care delivery and compliance with organisational and regulatory requirements.

 

Reporting to the Nurse and Healthline Lead, you will:

  • Deliver advanced, evidence‑based clinical nursing care and case management
  • Exercise high‑level autonomous clinical judgement and decision‑making within scope of practice
  • Contribute to clinical standards, protocols and service improvement within a community‑based, nurse‑led model
  • Coordinate and prioritise care using advanced clinical reasoning, including timely referrals across multidisciplinary and community services
  • Deliver, monitor and evaluate therapeutic interventions and support programs
  • Collaborate within multidisciplinary primary health care teams
  • Advocate for clients and carers to support equitable access to health, Aged Care and NDIS services
  • Build and maintain strong community and professional networks
  • Provide culturally safe, inclusive care that promotes access and equity
  • Deliver education to health professionals, clients, families and stakeholders
  • Maintain advanced clinical expertise through ongoing professional development and a contemporary practice portfolio
  • Contribute to research, education resources, data collection and quality improvement activities

About you

  • AHPRA Registered Nurse with a minimum 5 years’ post graduate experience
  • Experience and or interest in chronic disease management and movement disorders
  • An understanding of primary and tertiary health care
  • Ability to work effectively both independently and as a member of a multi-disciplinary team with a flexible positive attitude.
